Ancillary Service Certificate Application (AS) - (Rev. 6/1/2024)
By completing this application, applicants may request an ancillary service certificate. This is a stand-alone certificate issued to individuals holding qualifying degrees or specialized credentials. The certificate allows one to serve in a Louisiana school system setting. This application packet does not include Ancillary Teaching, Ancillary Early Childhood, Mentor Teacher, or Content Leader certification; there are separate ancillary application packets for those certification requests. Ancillary certification to serve as a School Counselor or Librarian would be requested on the Ancillary Teaching Certification request since those are renewable with effectiveness, like teaching. Upon receipt of the completed forms and appropriate processing fee as noted on the Certification Processing Fee Schedule, a certification case will be opened in the Teacher Certification Management System (TCMS) which is trackable online. If additional information is needed or feedback is provided, that information can be viewed online by checking the Status of a Certification Application here. Evaluations will be provided via the online portal in response to the certification submission case.

Ancillary Teaching Certification Application (AT) - (Rev. 6/1/2024)
By completing this application, applicants may request an ancillary teaching certificate. This is a stand-alone certificate issued to individuals holding qualifying degrees or specialized credentials. The certificate is renewable with effective evaluations and is requested by the Louisiana employing school system on behalf of the educator. This application packet does not include Ancillary Service, Ancillary Early Childhood, Mentor Teacher, or Content Leader certification; there are separate ancillary application packets for those certification requests. Upon receipt of the completed forms and appropriate processing fee as noted on the Certification Processing Fee Schedule, a certification case will be opened in the Teacher Certification Management System (TCMS) which is trackable online. Appeals Application (Rev. 6/1/2024)
The certification appeal process is available to an individual who has applied for certification and has been denied the requested certification due to the absence of certification requirements as set forth in Bulletin 746, Louisiana Standards for State Certification of School Personnel. The appeal process provides the applicant an opportunity to have his/her records reviewed by the Teacher Certification Appeals Council (TCAC). Do NOT submit an appeal via the online portal; email it to CertificationAppeal@la.gov. The appeals application requires a $25.00 processing fee.

Certification and Exam Verification (VR) - (Rev. 6/1/2024)
By completing this application, educators may request official Louisiana certification and/or exam verification. Verification will be provided via the online portal in response to the certification submission case and can be sent to other agencies if noted in the request. A $25 handling/processing fee is required for this type of request. Certification may be officially verified online for no charge at www.teachlouisiana.net. Louisiana’s certification structure and policies are outlined in Bulletin 746, Louisiana Standards for State Certification of School Personnel. All questions regarding certification requirements or the process, can be answered by contacting the certification staff through the online educator certification portal at ldoe.force.com.

This application packet is used to facilitate the process of RENEWING the Louisiana Early Childhood Ancillary certificate. Louisiana’s Early Childhood Ancillary certificate authorizes an individual to teach in a publicly-funded early learning center serving children age birth to five as defined in R.S 17:407.33.

Educational Leader Application (Rev. 6/1/2024)
The Educational Leader (EDL) certificate is needed by those who fill school and district educational leadership positions (e.g., assistant principal, principal, parish or city supervisor of instruction, supervisor of child welfare and attendance, special education supervisor, or comparable school/district leader positions). This certificate is issued upon the request of the Louisiana employing school system once the individual is hired to serve as an educational leader. The EDL stand-alone certificate will be issued upon verification from a Louisiana employing school system that individual has been hired/employed as an educational leader (e.g., principal, asst. principal, etc.). If verification is not submitted, a letter of eligibility for requested endorsement will be issued to applicant.

Educators may request degree/endorsement additions, course/transcript evaluations for the purposes of fulfilling add-on, and course analysis/pre-approval.
Name changes are included as a free transaction on all applications as the SS card and driver’s license are required with ALL certification applications. If the name change is requested with no other transaction, this application packet is used and a $25.00 fee is required.

Provisional Mentor Teacher (ancillary) certification allows one to serve as a mentor to teacher-candidates in undergraduate and alternative certification programs while completing the requirements for full mentor teacher certification.
Content Leader (add-on and ancillary) certification allows one to serve as a content expert in their Louisiana employing school system.

Non Public Temporary (T) Application (Rev. 6/1/2024)
Louisiana offers the Non-public Temporary (T) certificate to teachers practicing in a nonpublic school setting who need temporary credentialing to meet nonpublic school standards as outlined in Bulletin 741 (Non-public), Louisiana Handbook for School Administrators. The T certificate is valid for one year and is renewable while the holder works towards meeting nonpublic school standards. Once the teacher fulfills non-public standards, the non-public school reports the teacher as “meeting non-public standards,” but no additional certificate is issued.

Out-of-State (OS) Application (Rev. 6/1/2024)
The Out-of-State application packet is used by educators who have completed a teacher preparation program in another state/country to obtain a standard Louisiana teaching certificate with or without exams, with or without actually obtaining a certificate in the state/country where prepared as a teacher. (If prepared by a Louisiana teacher preparation program/university, even if the educator has obtained certification in another state, the educator should contact the university/program provider for assistance in completing the Initial (IN) Application as the recommendation must be directly submitted by the university/program provider.)

Out-of-State (OS) for Military (OSm) Application (Rev. 6/1/2024)
The Military Out-of-State application packet is used by educators who are active military or military spouse applicants and who hold standard professional-level teaching certification in another state to obtain a standard Louisiana teaching certificate.
Applicants who have completed a teacher preparation program, including student teaching or equivalent, in another state but do NOT hold standard professional teaching certification in another state should review and submit the standard Out-of-State (OS) Application.

Certification options for educational leaders who have NOT completed Louisiana’s licensing exams but who have leadership experience and certification in another state can apply for the Out-of-State Principal (OSP) or the Out-of-State Superintendent (OSS) certificate to serve as an assistant principal, principal, assistant superintendent, or superintendent.

Out-of-State Experience Verification Attestation (Rev. 6/1/2024)
Per Louisiana statute, R.S. 17:3886, educators seeking regular teacher certification in Louisiana who hold a teacher certificate from out-of-state and have out-of-state teaching experience shall not be credited with said experience until receipt of evidence of successful teaching experience.
To provide evidence of successful out-of-state teaching experience, this form must be completed and signed by the appropriate out-of-state entity: 1.) Out-of-state credentialing agency (department of education, etc.), or 2.)Out-of-state employing school system.

Professional Conduct Form - (Rev. 8/30/2023)
This form is part of all certification application packets and is required to be completed/signed/dated/submitted with every certification request. All questions must be answered truthfully as a matter of ethics accountability and if any responses are checked "yes," relevant, explanatory documentation (e.g. court minutes, legal documents, BESE or LDOE letters/communication regarding the matter) is required to be submitted with the application packet.

World Language Certificate (WLC) Application (Rev. 6/1/2024)
Louisiana offers World Language Certificates (WLC, formerly FLS/FLES Certificate) to applicants participating in the Louisiana Foreign Associates Program.
The WLC is valid for six years and can be renewed with effective evaluations.
