What must be included in correspondence sent to the certification office?
All correspondence sent to certification should include the following:
- Full name and date of birth
- Social Security Number
- Certificate number (if known)
- Valid Email address (this is where an electronic notification of your certification request will be sent)
- Mailing address
- Telephone number
- With all application requests, a certification fee and a Professional Conduct Form (which is part of any of the application packets found here) should also be included.
