Certification

What must be included in correspondence sent to the certification office? All correspondence sent to certification via the online portal should include the following:
  • Full name and date of birth
  • Social Security Number
  • Certificate number (if known)
  • Valid Email address (this is where an electronic notification of your certification request will be sent)
  • Mailing address
  • Telephone number
  • With all application requests, a certification fee and a Professional Conduct Form (which is part of any of the application packets found here) should also be include
Work with your employing school system to ensure the proper documentation is uploaded.  Policy may require the request be submitted by the Louisiana employing school system.
