What must be included in correspondence sent to the certification office? All correspondence sent to certification should include the following:
  • Full name and date of birth
  • Social Security Number
  • Certificate number (if known)
  • Valid Email address (this is where an electronic notification of your certification request will be sent)
  • Mailing address
  • Telephone number
  • With all application requests, a certification fee and a Professional Conduct Form (which is part of any of the application packets found here) should also be included.
